A detailed Medical Case Review and an In-Office Interview/Physical Examination will be conducted. Based upon thorough analysis, we will provide a written report that includes a listing of the Pertinent Records reviewed, Injury History, Chronologic Summary, Graphic Medical Timeline, Past Medical History, Physical Examination, and a Current Diagnostic Assessment. The discussion will include reviewing the Clinical Evaluation and Treatment, characterization of Non-organic vs. Organic findings, Causation Issues, and Prognosis/Impairment.
